Instructions for Completing the BizToBiz Associate Application

  1. Fill out the Associate Application: Please provide all of the requested Personal and Contact information and answer the questions in the Background Questionnaire. You can go “Save” or “Exit” the application at any time and complete the application later.
  2. Accept the Terms of Use For BizToBiz Website: Please review and accept the terms of use for BizToBiz Website.
  3. Enrollment Processing Fee – $125.00:

Before you proceed, please make sure you have the following information:

  1. Your Social Security Number. Biz To Biz Insurance Solutions LLC. and its affiliates and subsidiaries are required by Federal law to obtain your Social Security Number tax reporting purposes. The number you provide must correspond to your legal name.
  2. Your Personal and Contact Information. This includes your current address, e-mail address, phone numbers, date of birth, and state Driver License information.
  3. Credit Card Information. You will be asked to pay a non-refundable $125.00 enrollment processing fee as part of the Application. Please have the exact name on your credit card, expiration date, and billing address ready.

  2. Due to licensing requirements and different legal requirements in various jurisdictions, we recommend that you do not apply unless you are a U.S. Citizen or Permanent U.S. Resident. In addition, please note that we are unable to accept Applications from individuals residing in the following jurisdictions: Alaska, Guam, Puerto Rico, U.S. Virgin Islands. This list is subject to change without notice.
  3. Associates are not compensated for recruiting other Associates. Compensation to Associates is based on commissions or referral fees for the sales of products and services offered to the public. In the event that your application is declined or canceled, you will not be charged the Enrollment Processing Fee.

Biz To Biz Insurance Solutions LLC and its affiliates and subsidiaries are not responsible in any manner for damage or injury that may occur from use of this site or the interception of your personal data by a third party. The Enrollment Processing Fee of $125.00 is non-refundable and covers the costs associated with creating and providing ongoing access to BizToBiz Website, and providing administrative support for applications and Insurance Carrier appointments (if applicable). Continued access to your account will be subject to the Terms of Use for this site. Attempts to refund or chargeback this processing fee will result in a revocation of access. Please note that you may also be responsible for any State examination fees, pre-licensing training and education, as well as any regulatory licensing fees (if applicable) and the Enrollment Processing Fee does not cover those costs.
